Notes for Customer Data Update by Foreign Nationals (New UI No.(ARC))

親愛的客戶，您好
為因應「內政部推動外來人口新式統一證號換發政策」，若您已換發新式統一證號者，提醒您下列事項：
Dear Customer,
In compliance with the “Policy of the Ministry of the Interior to Promote the New UI No. Format for Foreign Nationals”, if you have renewed the unified certificate number into the new format, please be reminded of the following:

統一證號資料變更說明（Explanation for the Change of Unified ID Number）

1. 請填寫「契約內容變更申請書」經要保人及變更者親簽，並檢附「最新身分證件(居留證)」影本，交由您的服務業務員送件或郵寄至本公司任一營業單位，辦理變更統一證號作業事宜。                                                                                                                                                                                                      Please fill in the “Application Form for Policy Change” signed by the applicant and the person who makes the change, with a copy of the “latest identity certificate (residence permit)” attached, assist by your service agent to apply or mail them to one of our business units to update your Unified ID Number.
2. 請您本人攜帶換發之「最新身分證件(居留證)」正本，偕同要保人親臨本公司任一營業單位辦理變更統一證號作業事宜。                                                    Please bring the original copy of the renewed “latest identity certificate (residence permit),” and visit one of our business units with the applicant to handle the update of the Unified ID Number.

注意事項（Notes）：

1. 如續期繳費方式約定為金融機構轉帳者，若您完成統一證號變更後需填寫「保險費付款授權書」重新約定。並洽原交易銀行同步變更您的統一證號，才能順利完成與該單位約定之扣款作業。                                                                                                                                                                                                If the agreed payment method for the renewal is payment via a financial institution, after completing the update of the Unified ID Number, you need to fill in the “Insurance Payment Authorization Agreement” to reset the agreement. And please contact the original transaction bank to change your Unified ID Number in order to successfully complete the debit agreed with that unit.
